NATURE OF WORK

As an Aging Program Assessor, you will gather pertinent assessment data through interviews with consumers and families, review medical and other records, and consult with medical personnel and other professionals. This information is used to complete comprehensive assessments of consumers needs, which include determining the most appropriate level and type of care needed, with the goal of maintaining consumers’ independence for as long as possible.

Aging Care Management Supervisors oversee the delivery of a range of services provided to consumers by Area Agencies on Aging and contractors. You will review and analyze case records for completeness and adherence to standards. You will also train and supervise staff, prepare reports, and collaborate with nurse consultants, other professionals, and various state and local organizations involved in social services.

Aging Care Management Supervisors 2 and 3 have a larger scope of responsibilities which may include identifying and resolving administrative problems, supervising the protective services program, planning budgets, and developing local policy.
