THE NOMINATION PAPER FORMS ON THIS PAGE ARE BEING PROVIDED FOR THE PURPOSE OF SUPPLEMENTING THE SUPPLY OF NOMINATION PAPER FORMS THAT YOU RECEIVED FROM THE BUREAU OF COMMISSIONS, ELECTIONS AND LEGISLATION. BECAUSE THESE FORMS MUST BE ACCOMPANIED BY ADDITIONAL DOCUMENTS AT THE TIME OF FILING, YOU MUST REQUEST AN OFFICIAL PACKET OF FORMS AND INSTRUCTIONS, IF YOU HAVE NOT ALREADY DONE SO.

THESE FORMS ARE FOR CANDIDATES OF INDEPENDENT POLITCAL BODIES. THEY CANNOT BE USED TO GAIN ACCESS TO THE DEMOCRATIC PARTY PRIMARY OR THE REPUBLICAN PARTY PRIMARY.

BEFORE DOWNLOADING OR PRINTING ANY OF THE NOMINATION PAPER FORMS ON THIS PAGE, PLEASE READ THE FOLLOWING INSTRUCTIONS VERY CAREFULLY. FAILURE TO REPRODUCE THE NOMINATION PAPER FORMS CORRECTLY MAY RESULT IN THE REJECTION OF THOSE FORMS.

  1. The nomination paper form (DSBE PB-G) must be printed as duplex (two-sided, single page, front and back) form on 8 ½" x 14" (legal size) paper. This requirement cannot be satisfied by printing each side of the duplex nomination paper form on a separate sheet of 8 ½" x 14" paper and affixing the two sheets together.
  2. The nomination paper form (DSBE PB-G) must be printed on plain white 8 ½" x 14" (Legal-Size) paper.
  3. Please read all instructions provided with the nomination paper forms prior to circulating them.
