2009 AYP Appeal Procedures
Directions for Filing an Appeal
There are four, separate Appeal Forms: a District Appeal Form, a School Appeal Form, a Charter School Appeal Form, and a Full-time CTC/AVTS Appeal Form. If you are a district, please do not submit an appeal for the district on a "School" Appeal Form. Similarly, do not submit an appeal for a school on a "District" Appeal Form. Appeals that are submitted on the wrong form cannot be reviewed. If you file an appeal for more than one school in a district, you must use a separate form for each school. Appeals postmarked after August 3, 2009, cannot be accepted. Please submit Appeals as a packet to include the Cover Page, Routing Page, all applicable Appeal Forms, and all supporting documentation. Do not submit pages separately.
