  • Applying for ACP Participation

    Applications are available from Pennsylvania victim service, domestic violence and/or sexual assault programs. Because the ACP is not for everyone, we encourage you to apply at a local(Need help? Find victim service programs in your area) program to help determine if the ACP should be part of your safety plan. If you need help finding a program in your area, please use the "Title of the county map". You may also call the ACP to request an application be sent directly to you.

    The application packet includes:

    • ACP Application
    • ACP Checklist
    • ACP Authorization Card

    You will also need to prepare an affidavit, a written or typed statement on a separate sheet of paper, describing past, present and fears of future violent acts by the perpetrator. The affidavit must be included with the application you send to the ACP. Please take your time and fill out the information as completely as possible.

    Although you may not apply online, the application online instructions can assist you in completing the paper application. A victim advocate from a local victim service, domestic violence and/or sexual assault program will also be able to help you.
