Conference Information 
Fee: Conference registration is $200 per person and includes admission to the conference workshops, exhibits, lunches, monring/afternoon breaks (on June 11-12), as well as dinner June 12.
Register by
credit card online, or complete the
registration form and send it,
no later than May 29, 2012, with your check or money order for
$200, payable to the
Commonwealth of PA to:

Provide all information requested on the registration form, print it out and return it by mail no later than May 29, 2012 to the address above.
Cancellation Policy: Cancellations and refund requests must be received by the bureau no later than one week prior to the event. No refunds will be made after this date. There will be no exception to the refund policy.
Walk-in registration: Walk-in registrations will no longer be accepted without contemporaneous payment. NO credit card payments on site.
Lodging/Reservations: Reservations for overnight accommodations at the Hershey Lodge must be made by contacting the Lodge at 717-533-3311 or 800-533-3131 or visit:
http:www.hersheypa.com/accommodations/hershey_lodge/ .When making your reservations, advise the Lodge that you are attending the Workers' Compensation Conference to get the conference single/double room rate of $189, plus tax. If you are attending
and exhibiting, make the Lodge aware of that as well..
