PDE 338 E Emergency Permit/Act 97 Waiver Agreement

Effective July 1, 2010, the fee for Type 01, 04 and 08 Emergency Permit applications is $100. The fee for Type 06 Emergency Permit applications if $5.

Note: Forms may be used for documentation/verification purposes only. All permit request applications must be submitted online through the Teacher Information Management System (TIMS). Applicants should not submit a permit request application in TIMS unless requested to do so by a school entity.
