Diabetes Control Program Initiatives

The Diabetes Control Program focuses on three major goals:

  • to improve the lives of Pennsylvanians with diabetes,
  • to reduce disparities related to diabetes among minority and underserved populations in the Commonwealth and
  • to prevent the onset of type 2 diabetes through reduction of risk factors. Program services are offered on a statewide basis supporting contract activities with state and federal funding.

The Diabetes Control Program underwent a strategic planning process and a shift of focus from a medical model to a population-based approach focusing on prevention. In order to facilitate this shift, the program identified seven major objectives:

  • Increasing awareness of diabetes
  • Preventing the onset of type 2 diabetes through reduction of risk factors
  • Reducing diabetes complications
  • Increasing the early diagnosis of persons with diabetes
  • Increasing the availability of programs that address psychosocial issues associated with diabetes
  • Increasing advocacy and empowerment of persons with diabetes
  • Maintaining a data and surveillance system to better understand the impact of diabetes in Pennsylvania
