Department Notice 2010-08 and Application. This application form is used by individuals who, because of a criminal felony conviction involving dishonesty or a breach of trust, are prohibited by The Violent Crime Control and Law Enforcement Act of 1994, 18 U.S.C. §§ 1033 and 1034, from engaging or participating in the business of insurance without the specific written consent of the Pennsylvania Insurance Department.
