CHANGE OF OWNERSHIP (CHOW)

Changes of ownership may occur at drug and alcohol facilities licensed by the Department of Health. 28 Pa. Code §§ 709.14(b)(1) and 711.14(b)(1) require that the Division of Drug and Alcohol Program Licensure be notified of any ownership changes within 90 days of the occurrence.

New Owner Information

List of information to be submitted by the new owners of the drug and alcohol facility(ies)
New owners are also required to submit a signed “Statement of Compliance” form.
