ELECTRONIC PLAN OF CORRECTION
On May 13, 2006 the Division of Drug and Alcohol Program Licensure implemented the electronic plan of correction process for drug and alcohol treatment facilities. Facilities are now able to receive and respond to deficiencies from on-site inspections through a secure website.
Survey results from inspections conducted after Jan. 1, 2005 are available for public viewing on this web site. For facilities participating in the electronic plan of correction, a response for inspections conducted after May 15, 2006 will be available via this website. Responses prior to this date are not available via the website but they are on file with the division.

Password Security Agreement for Facility Directors
- After you are notified by the division that your email registration request has been accepted, submit a Password Security Agreement for Facility Directors Form. Please do not submit this form before being notified by the division that your email registration request has been accepted.
