How To Correct Personal Information on a Fetal Death Certificate when Delivery Occurred Within Past Two Years
To correct spelling or typographical errors in the name of the stillborn fetus or parent’s information, it will be necessary for the funeral director who signed the original death certificate to complete a Death Correction Statement. Please list the information in the “Correct” column and hand sign in the designated area. The individual requesting the correction must include a legible copy of his/her valid government issued photo ID.

To correct spelling or typographical errors in the name of the stillborn fetus or parent’s information, it will be necessary for the funeral director who signed the original death certificate to complete a Death Correction Statement. Please list the information in the “Correct” column and hand sign in the designated area. In addition, a photocopy of any one document that conclusively establishes the information to be corrected must be submitted. Acceptable documents include a hospital or physician’s record. The individual requesting the correction must include a legible copy of his/her valid government issued photo ID.
Major Changes to the Name, Date of Delivery or Cause of Death
It will be necessary to contact the physician or medical examiner/coroner who signed the original fetal death certificate for assistance in changing these items on the fetal death certificate.
If Correction To Fetal Death Certificate Is Not Addressed Above
If the correction you desire is not addressed above or you require additional information, you may contact the Division of Vital Records by mail. Please submit a completed application form and specify the correction(s) desired to the fetal death certificate. In addition, a photocopy of any one document that conclusively establishes the information to be corrected may be submitted for our review. The individual requesting the correction must include a legible copy of his/her valid government issued photo ID with the completed application form and the required fee or Armed Forces information.
