Fill-in Form Security for Shared Computers/Workstations
The Department of Revenue provides fill-in forms as a convenience to our taxpayers.
If you are using a shared computer or workstation, please take the following step to protect your privacy.
After you print your completed form, go to the very end of the document and click the Reset Form button. The data you have typed (name, social security number, etc.) is stored in a cache for use in other forms. Clicking the Reset Form button clears all of your entries. Until you clear them, they remain in a cache and may be viewed the next time the form is opened on the same machine.
The reason it is necessary to clear the form is that Adobe Acrobat Reader has a feature allowing data to be entered once and used to fill multiple forms. This is not due to a flaw in the Pennsylvania Department of Revenue forms.
