The Sign language Interpreter Registration Act, (Act 57) has eight exemptions, outlined in Section 1725.4. The following two exemptions require a form to be completed:

Allows interpreters certified by the Registry of Interpreters for the Deaf (RID) or the National Association of the Deaf (NAD) who do not reside in the commonwealth to provide interpreting or transliterating services within Pennsylvania for a period not exceeding 14 days each calendar year. To comply with the act and its regulations, interpreters are required to complete the Exemption Form for out-of–state interpreters working in PA (ODHH-5).
This exemption only applies to physicians’ offices. Patients who are deaf have the right to accept the services of a nonregistered interpreter or request a registered interpreter. If the patient chooses to accept the services of a nonregistered interpreter, all parties should complete the Request for Use of Non-Registered Interpreter/Transliterator in Physician’s Office form (ODHH-4).
