In order to obtain view-only access to the PA-SIIS, each medical staff member of your school will need to review the PA-SIIS Confidentiality Policy and then complete a PA-SIIS User Agreement Acknowledgment(School) Form.

All completed PA-SIIS User Agreement Acknowledgment forms for your school should be mailed together to the following address:

After you have completed and submitted the User Agreement Acknowledgment forms, a PA-SIIS staff member will mail your account information to your school.
