Welcome to our website!
The Special Kids Network (SKN), in partnership with the Pennsylvania Elks Home Service Program, helps children and youth with special health care needs and disabilities get the services and support they require in order to thrive in their community and develop to their full potential. We accomplish this by:

offering service coordination to families through a partnership with the Pennsylvania Elks Home Service Program where an Elks Home Service staff person will work with families in their homes to connect them to needed services and resources; and,
-
supporting six regional coordinators, who are all parents of children with special health care needs and disabilities, to work in communities to identify service gaps and address the unique challenges of service systems.

 The Pennsylvania Department of Health, along with other commonwealth agencies and community partners, recognized the gaps in health and other services for transitioning youth and those with special health care needs. When youth with special health care needs move toward adulthood, they face changes dealing with health care providers, insurance and other services. Working with the Pennsylvania Community on Transition State Leadership Team, agencies and families, the Transition to Adult Living in Pennsylvania: Transition Health Care Checklist was created to help guide youth, families and professionals in planning during this time of transition.
