Board Services
Rules and regulations - A hard copy is available from the Board office upon request. Remember to include your mailing address with your request.
Name change - Will be made only when a copy of an official document (marriage certificate, divorce decree or court order) is submitted with a letter of request. The fee for a duplicate license is $5.00 (check or money order) payable to the Commonwealth of PA.
Address change - Written requests should include the old address, new address and license number and/or social security number. The Board must be notified within 10 days of any change in address.
Certification of Licensure to Another State - A $15.00 check made payable to the "Commonwealth of PA" is required for each certification, along with a written request including your name, license number and address of the other state licensing board. Your fee and information should be sent to the State Board of Dentistry, P.O. Box 2649, Harrisburg, PA 17105. |

Continuing Education Credits are required for each biennial period. The biennial period runs from April 1 to March 31 of the odd-numbered years. The requirements are as follows:

*Dentists - 5 hours (Anesthesia): Dentists who practice in an office where other individuals administer general anesthesia, deep sedation or conscious sedation who do not hold an Anesthesia Unrestricted or Restricted Permit I are required to complete 5 hours of anesthesia related courses in accordance with the Board's Regulations.
NOTE: The 15 hours of continuing education required to maintain the Anesthesia Unrestricted or Restricted Permit I and the 5 hours of continuing education required for dentists who do not hold the Unrestricted or Restricted Permit I who practice in an office where other individuals administer general anesthesia, deep sedation or conscious sedation can be applied to the 30 hour continuing education requirements to maintain the license to practice dentistry.
NOTE: The 3 hours of continuing education required to maintain the Dental Hygiene Local Anesthesia Permit and the 5 hours of continuing education required to maintain the Public Health Dental Hygiene Certification can be applied to the 20 hour continuing education requirements to maintain the license to practice dental hygiene.
