Rules and Regulations - A hard copy is available from the Board office upon request.  Remember to include your mailing address with your request. 
 
Name Change - Will be made only when a copy of an official document (marriage certificate, divorce decree or court order) is submitted with a letter of request.  The fee for a duplicate license is $5.00 (check or money order) payable to the Commonwealth of PA.
 
Address Change  - Written requests should include the old address, new address and license number and social security number.  The Board must be notified within 10 days of any change in address.

On March 14, 2009, the forms required by the Long-Term Care Patient Access to Pharmaceuticals Act were published in the Pennsylvania Bulletin. The Act takes effect in 90 days from that date.

Board Examination Information:
 
North American Pharmacist Licensure Examination (NAPLEX) and Multistate Pharmacy Jurisprudence Examination (MPJE). 
 
For examination applications contact:  State Board of Pharmacy, P.O. Box 2649, Harrisburg PA  17105-2649.  Phone 717-783-7156  FAX 717-787-7769 .  Completed license applications are submitted directly to the State Board of Pharmacy. Applicants may register to take the examinations at NABP.
 
The NAPLEX and MPJE are computer-based examinations and deadlines vary.

License Renewal Cycle and Fees: 
 
The board mails renewal notices 2 to 3 months prior to the license expiration date.  Notices are mailed to the most recent address the licensee has reported to the Board.  The Postal Service does not forward licenses.

Proof of completing 30 contact hours of continuing education in programs approved by the Accreditation Council for Pharmacy Education (ACPE) is required for renewal of pharmacist licenses. For pharmacists with the authorization to administer injectable medications, biologicals and immunizations, at least 2 of the required 30 contact hours shall concern the administration of injectable medications, biologicals and immunizations, including disease epidemiology, vaccine characteristics, injection technique, emergency response to adverse events and related topics. Starting October 1, 2012, 2 of the 30 contact hours required for the renewal of a pharmacist license shall be completed in the ACPE topic designator "Patient Safety."
