A W-9 form is an IRS form used by a company to request your Taxpayer Identification Number (TIN) and certification. The TIN and Legal Name provided during the registration process must match the W-9 in order to receive a contract or payment. The Payable Services Center will validate that a match exists between the registration and the W-9 form. |

Manage Company ACH Banking Information |
- Another critical step in the Commonwealth's validation process is the entry of accurate bank information in the Edit Bank Data & Product Categories page, accessible to logged in Administrative Users. As a supplier capable of receiving payments, this information MUST be completed prior to submission of the W-9 form. You can access this page on the left navigation after you are logged onto the PA Supplier Portal.
- Please ensure the ACH Banking Information is accurate prior to submission of the W-9 form.

- The following information is required to complete the W-9 Form:
Your Name and Social Security Number OR the Legal Name and Employer Identification Number (equivalent to TIN) of your business.
- After the form is completed, please click on the print icon to print the document, sign and fax the form.
- Fax completed W-9 forms to the Payable Services Center at(717) 214-0140 (fax number only).

Companies involved in a MERGER or ACQUISITION: |

1. Determine if your company was involved in a Merger or an Acquisition:
- Merger
A merger occurs when two companies combine to form a single company. In a merger, existing stockholders of both companies involved retain a shared interest in the new corporation. In this type of transaction, your Employer Identification Number (equivalent to TIN) does not change.
- Acquisition
The process of gaining control, possession or ownership of a private portfolio company by an operating company or conglomerate. The Employer Identification Number (equivalent to TIN) of the Acquired Company will change.
2. The Customer Services Call Center will assist you in changing your company's profile information on file with the Commonwealth of Pennsylvania. |
