Volunteer Fire Company, Volunteer Ambulance Service Grant Final Report Form

FINAL REPORT FORMS MUST BE FILED PRIOR TO DECEMBER 1, 2011

The Volunteer Fire Company, Volunteer Ambulance Service Grant Agreement that all applicants signed in order to receive their 2010-2011 grant awards includes a requirement that recipients file a Final Report regarding their project expenditures.

According to the Volunteer Fire Company and Volunteer Ambulance Service Grant Program Statement of Policy for the combined 2010-11 grant cycle, that Final Report is to be submitted no later than December 1, 2011.  Projects must be paid for between July 1, 2010 and December 1, 2011 to comply with the requirements of the 2010-11 grant.

As a practical matter, 2010-11 grant recipients should submit their 2010-11 Final Report as soon as they complete their projects. Based on the amended grant legislation, the Office of The State Fire Commissioner will NOT include any Volunteer Fire Company or Volunteer Ambulance Service Grant applications in the 2011-12 VFCVAS Grant calculations if that company has not fulfilled the requirement that they submit a Final Report documenting the expenditure of funds they received from the 2010-11 grant program prior to December 1, 2011.

Depending on the speed of your computer and internet access, the fill-in application could take some time to download.

Adobe Reader does NOT allow you to save your filled-in application. When you close the form all information you have entered will be lost. You should therefore complete the form with all the information requested and then print two copies, one for your records and one to sign and send to the OSFC as instructed.
