When the 2012-13 VFCVAS Grant Program application period closed on October 21, 2012 there were 2,713 eligible fire and EMS organizations that submitted applications for grant funding. The OSFC staff completed their review of these applications and 2,665 applications were approved. The total amount of all grant awards requested was $37,521,329. The final distribution to Fire Companies totaled $26,400,000 and for Vounteer Ambulance Services a total $3,600,000.

ACCESSING YOUR GRANT AGREEMENT FORMS
Grant agreement forms for the 2012-13 Fire Company, Volunteer Ambulance Service Grant Program will be available on Monday, January 7, 2013.
Applicants that applied for the grant using the Online Grant Application can access their agreement form online. If you have forgotten your password, please enter your USER ID at the FCFAS grant program log in and "click here to reset your password". A temporary password will be sent to the email address listed in your profile. If you have forgotten your USER ID please send an email to firstname.lastname@example.org and you will receive a response.
As in previous grant programs, there is the familiar "Complete Agreement" form that applicants should save for their records. It contains all the details about your grant and the regulations that govern it. Please print or save a copy of this Complete Agreement for your records.
There is a "Signature Section" for this Agreement Form which serves two purposes:
This form is the document which must be signed and returned to the Office of the State Fire Commissioner (OSFC) certifying that your organization is aware of, and agrees to the terms of your 2012-13 FCVASGP grant. It must be signed by an authorized officer of your organization. Your grant cannot be processed without returning this signed form. In addition, this form also serves as your organization’s authorization for the OSFC to deposit your grant funds to your account using “Electronic Funds Transfer” (EFT). You will have to take the form to your financial institution (bank, credit union, etc.) and have them complete and sign the “Financial Institution” section. Grant funds will only be disbursed using this direct deposit method.
IMPORTANT INFORMATION REGARDING BANK CHANGES: The Vendor Data Management Unit (VDMU) now requires that any bank changes to your VDMU record must be made by submitting their Pennsylvania Electronic Payment Program (PEPP) Enrollment Form and a copy of a blank/voided check. Complete the PEPP form and return to our office along with your signed grant agreement if you have changed your bank information or are a new grant recipient. This includes any bank mergers in which the Bank Routing Number has changed. If this information is not received, by our office, with your original signed grant agreement it will delay processing your grant payment. DO NOT CONTACT OUR OFFICE, it delays processing of grant agreements and we are unable to verify bank information. If you are unsure, do not know, or believe this information has changed, please submit this form with a voided blank check along with your signed agreement.
IMPORTANT INFORMATION REGARDING GRANT AMENDMENTS: The grant amendment process is now completed on-line. Please review the enclosed grant amendment information when completing a grant amendment. You must complete an amendment for any project(s) not listed on your original grant application as our guidelines have changed. Grant Amendment Instructions

After you have read the instructions, please log on to the ONLINE GRANT APPLICATION. To access your agreement forms Click on the "Click here to access the Online Grant Application" bar.
When all the required information and signatures are completed, return all the pages of the Signature Section of your Grant Agreement (please do not staple grant agreement) to:
