Applying For a Birth Certificate For Voter ID
Please note: If you do not have a birth certificate with a raised seal and are a Pennsylvania native; and do not have one of the acceptable, alternative forms of photo identification to vote; please visit the Pennsylvania Department of Transportation website for additional information.
In conjunction with Act 18 of 2012, Pennsylvania citizens may request a birth certificate for Voter ID. The certificate will be issued free of charge. In order to receive a certificate, each requestor must complete the Voter ID birth certificate application form and mail it to one of the below public offices. Individuals may also apply in person at one of the public offices during normal business hours.

Voter ID Refund
If you have already applied and paid for your birth certificate for VOTER ID purposes, you may apply for a $10 refund. To do so, complete the Voter ID Refund Form and mail it to:

The certificate must have been issued from 3/14/2012 through 6/12/2012.
