The following manufacturers’ labeler codes have been reinstated as participants in the Program’s manufacturers’ rebate program:

Labeler Code 00378 Mylan Pharmaceuticals Inc.

Labeler Code 51079 UDL Laboratories Inc.

The effective date for Mylan and UDL’s reinstatement is Monday, February 9, 2004.

Since the program permits original claims to be billed within 30 days from the date of service, providers may submit these claims from February 9 through February 17, 2004, to the Program for processing.
