PACE
Patient Location Definitions
At
the request of PACE Providers, we are providing the following definitions to
assist in the determining the number to be entered in NCPDP v5.1 field 307-C7,
Patient Location.
0 = Not Specified. This value will deny as Invalid.
1 =
Home—Place where the cardholder resides other than nursing home or other
facility. May be the cardholder’s residence or that of relative.
2 = Inter-Care— No current NCPDP Definition.
3 = Nursing Home—A facility licensed by the
state which provides skilled nursing care and related services for
rehabilitation of their residents. This facility accepts the responsibility for
the dispensing and monitoring of prescription medication.
4 = LongTerm / Extended Care—A facility,
licensed by the state, which may include a nursing home or skilled care that
accepts the responsibility for the dispensing and monitoring of prescription
medication.
5 = Rest Home—Assisted living/personal care.
Medication is stored and dispensed by the staff. Staff does accept
responsibility for dispensing and monitoring of prescription medication.
6 = Boarding Home—A state licensed facility;
not a private residence. Cardholder is
responsible for his/her own medication. Staff does not accept responsibility
for dispensing and monitoring of prescription medication.
7 = Skilled Care Facility— A state licensed
facility. Meets definition of 3 or 4 above.
8 = Sub-Acute Care Facility— A state licensed
facility. Meets definition of 3 or 4 above.
9 = Acute Care Facility— A state licensed
facility. Meets definition of 3 or 4 above.

= Outpatient—NCPDP definition. N/A for PACE/PACENET cardholders.

= Hospice—A facility, other than the patient’s home, which supplies palliative
and supportive care for terminally ill
patients. Medications should not be billed to PACE/PACENET.
