Instructions

Purpose:  A Veterinary-Client-Patient Relationship means that:

  • A veterinarian has assumed the responsibility for making veterinary medical judgments regarding the health of the animal and the need for veterinary medical treatment; and
  • The owner or caretaker of the animal has agreed to follow the instructions of the veterinarian in relation to zoonotic diseases.

Who must use the form?
Any person wishing to exhibit an animal at a show in Pennsylvania must provide a Veterinary-Client-Patient Relationship form. 

Meeting the Requirement To meet this requirement, a signed verification from the animal owner or caretaker, using the attached "Animal Owner or Caretaker's Verification of Veterinarian-Client-Patient Relationship" form must be completed by each exhibitor.     Upon arrival of an animal at an animal exhibition, a complete and signed relationship form must be collected from each exhibitor.  This requirement is in addition to the Certification of Veterinary Inspection (Health Certificate) or Herd Certification Requirements (llamas, poultry, rabbits and reptiles are exempt from the Health Certificate requirement).
