Purpose: A Veterinary-Client-Patient Relationship (VCPR) means that:
•A veterinarian has assumed the responsibility for making veterinary medical judgments regarding the health of the animal and the need for veterinary medical treatment; and
•The owner or caretaker of the animal has agreed to follow the instructions of the veterinarian in relation to zoonotic diseases.
Recent legislation in the form of PA Senate Bill 526 now requires few, if any, animal exhibitors to produce the VCPR form found on this website. Please see below for exemptions.
Who must use the form?
You Do Not Need to Use This Form If:
1. A Certificate of Veterinary Inspection is required for the animal or animals which you are exhibiting.
2. An Owner Endorsed Health Certificate (poultry) is required for the animal or animals which you are exhibiting.
3. The Fair Registration Form which you must fill out contains an Owner or Caretaker’s Verification of Veterinarian-Client-Patient Relationship verification statement.
Certification Requirements (llamas, poultry, rabbits and reptiles are exempt from the Health Certificate requirement).
