NATURE OF WORK
Community Health Nurse (State Job Title) - This is professional nursing in community settings involving the direction, coordination, and provision of varied community health services. Work includes direct delivery of clinical nursing services within the community, encouraging and facilitating the participation and/or development of new organizations, conducting needs assessments and program development, providing technical assistance and consultation, and conducting structured education and training.
Community Health Nurse Supervisor (State Job Title) - This is supervisory professional nursing in organizing, promoting and participating in the operation of public health programs. Work involves supervising the direct delivery of clinical nursing services, identification of staff development needs, evaluation and initiation of development training for staff, and completing and conducting performance evaluations.
Community Health Nurse 1 (Local Government) - This is professional nursing in identifying and assessing the physical and psychosocial needs and health status of individuals, particularly those receiving or seeking services through an Area Agency on Aging. Work includes completing consumer assessments, reviewing and endorsing assessments completed by other professional staff, providing co-workers with medical consultation and advice on particular cases, and maintaining collaborative relationships with others in the community involved with consumer care.
Community Health Nurse 2 (Local Government) - This is professional nursing in identifying and assessing the physical and psychosocial needs and health status of individuals, particularly those receiving or seeking services through an Area Agency on Aging. Work includes completing consumer assessments, reviewing and endorsing assessments completed by other professional staff, providing co-workers and managers with medical consultation and advice on particular cases, maintaining collaborative relationships with others in the community involved with consumer care and may involve supervising/leading the work of nurses and other members of a professional staff.
JOB REQUIREMENTS

You must be willing to travel. Travel expenses will be paid.
You must be a Pennsylvania resident, of good moral character and able to perform the essential functions of the job.
Some of the positions in these job titles come under provisions of a Child Protective Services Law. If you are a final candidate for a position covered by the Child Protective Services Law, you will have to provide reports on your background, at your own expense, from such sources as the Pennsylvania State Police and the Pennsylvania Department of Public Welfare.
Some of the positions in these job titles come under the provisions of the Older Adults Protective Services Act. If you are a final candidate for a position covered by the Older Adults Protective Services Act, you will have to obtain a report of criminal record information, at your own expense, from the Pennsylvania State Police, or a statement that the State Police Central Repository contains no information relating to you. If you have not been a resident of Pennsylvania for two years prior to the date that you received notification that you are a final candidate, you are also required to furnish a report of Federal Criminal History Record Information, at your own expense, from the Federal Bureau of Investigation.
Necessary Special Requirement For All Job Titles
Possession of a current license to practice as a Registered Nurse issued by the Pennsylvania State Board of Nursing; or possession of a non-renewable temporary practice permit issued by the Pennsylvania State Board of Nursing.
Condition of Employment For All Job Titles
Employees possessing non-renewable temporary practice permits must obtain licensure as a Registered Nurse within the one (1) year period as defined by the Pennsylvania State Board of Nursing.
Minimum Requirements
ALL Job Titles - Must possess the Necessary Special Requirement and meet the Condition of Employment.
30830 - Three years as a Community Health Nurse; OR four years of community health nursing experience; OR an equivalent combination of experience and training.
L1120 - A bachelor’s degree in nursing; OR an equivalent combination of experience and training.
L1121 - Two years of community health nursing experience and a bachelor’s degree in nursing including or supplemented by the required number of credit hours of community health nursing education; OR an equivalent combination of experience and training. (A master’s degree in nursing or public health may be substituted for one year of community health nursing experience.)
Clarification of Requirements
You may substitute related education for the required experience or appropriate experience for the required education. Unrelated experience or education will not be accepted.
30820 - You may take the test if you have completed 45 or more credit hours and expect to receive an associate's degree within the next 7 months. You must provide proof of your degree before you start work.
L1120 - You may take the test if you have completed 90 or more credit hours and expect to receive a bachelor's degree within the next 7 months. You must provide proof of your degree before you start work.

Community Health Nurse and Supervisor jobs are located in all counties of the state with the exception of Bucks, Chester, Erie, Montgomery and Philadelphia Counties.
Community Health Nurse 1 (Local Government) and Community Health Nurse 2 (Local Government) jobs are in the county Area Agencies on Aging located throughout the state.
Lists created by these tests will be used to fill future vacancies as they occur due to retirements, promotions, transfers, resignations and so forth.
Employment preference for Community Health Nurse 1 (Local Government) and Community Health Nurse 2 (Local Government) may be given to residents of the county where jobs are located. Promotion preference for Community Health Nurse 1 (Local Government) and Community Health Nurse 2 (Local Government) may be given to employees who work in the county where job openings occur.
Employment preference will be granted to individuals aged 60 and over for jobs with the county Area Agencies on Aging.

TESTING
You will be rated on your experience and training. Your score will be based on information you report on your application and application supplement.
You must provide your license number and expiration date of your Registered Nurse license in the “Licenses or Certificates” section of your Application for Employment/Promotion. You must provide complete and accurate information. Failure to do so may delay the processing of your application, or result in a lower-than-deserved score or disqualification.
If you take the test and want to take it again, you may be retested after 6 months from the date of your “Notice of Examination Results.” A new application and application supplement are required for retest.
If your name is on an eligible list for these job titles and you take this test, your most recent examination result will be the only one counted.
TEST RESULTS
Employment and promotion lists will be established. You will be notified in writing of your test results.
