Material Master Request Form
A material master record contains data of a descriptive nature (such as size, dimension and weight) and data with a control function (such as material type and industry sector).

Food Complaint Form
The Food Supplier Performance/Complaint/Rejection Reporting Form is available for Commonwealth agencies to quickly notify the DGS Commodity Specialist about current food delivery issues.  These can be complaints about the food item delivered including rejections, and complaints about how and when the item was delivered.

By hitting the “Submit” icon at the end of the form, the information provided will go into a database monitored by the Commodity Specialist.  From this information, the Commodity Specialist can determine if follow-up action is required with the food supplier.

Corrective Action Form (Now Available)
The Corrective Action Form is available for Commonwealth agencies to notify the QA Division about current issues concerning a supplier’s poor performance on a statewide contract.  This can also include the product delivered or service provided by the supplier.  If needed, please use the “Comments” section to further explain the corrective action issues.  If available, please attach additional documents and photographs. The areas marked with an asterisk (*) are required fields. 

By completing and hitting the “Save” button at the end of the form, the information is sent to the QA Division for review.  A Corrective Action Number is assigned and the form is sent to a QA Specialist.  The person submitting the form will receive notification of the Corrective Action Number and the name of the QA Specialist assigned to review the issue.

Supplier Performance Survey
Commonwealth agencies can use the following survey to rate current suppliers on statewide material and services contracts.
