DESIGN PROFESSIONAL SELECTIONS

FOR

DEPARTMENT OF GENERAL SERVICES

 

 

LETTER FROM THE DEPUTY SECRETARY FOR PUBLIC WORKS, DEPARTMENT OF GENERAL SERVICES


 

 

The Selections Committee for the Department of General Services will meet to consider selection of Design Professionals for the following projects:

 

PROJECT NO. DGS A 251-685 – Replacement of Equalization Tanks and Building Enclosure, PennDOT Roadside Rest Sites #17 & #18, Mercer County, PA.  Construction Cost:  $359,375.00.  The scope of work includes, but is not limited to, removal of the existing treatment plant’s influent bar screen and comminutor chamber.  Construction of a minimum 5,200 gallon (effective capacity), precast concrete inline flow equalization tank, prior to the treatment plant inlet.  Provide a positive displacement blower with motor and controls in an all-weather fiberglass enclosure.  Provide electrical service to the blower from existing control building if available and meets current codes or run a new service for blowers form the safety rest area.

Professional Design Basic Services Fees for this project have been established at 15% of the Construction Allocation.

PROJECT NO. DGS A 251-704 – New Vehicle Wash Building, PennDOT Maintenance District 8-7, Lancaster, Lancaster County, PA.  Construction Cost:  $502,015.00.  The scope of work includes, but is not limited to, construction of a new vehicle wash building.  The building shall be lighted for night work and heated for winter operation.  Proper drainage is a requirement of the design including all permits.  Building shall also include a heated enclosed area for the county brine making equipment and unheated storage.

Professional Design Basic Services Fees for this project have been established at 15% of the Construction Allocation.

PROJECT NO. DGS A 251-795 – Replace Roof Over Garage and Office Area, PennDOT Maintenance District 5-4 County Office, Stroudsburg, Monroe County, PA.  Construction Cost:  $390,156.69.   The scope of work includes, but is not limited to, replace existing EPDM roofing, insulation, flashing and other accessories for two garage roof sections of the existing building.

Professional Design Basic Services Fees for this project have been established at 10% of the Construction Allocation.

PROJECT NO. DGS A 251-893 – Multi-Building Demolition and Restroom Reconstruction, PennDOT Maintenance District 3-5, Selinsgrove, Snyder County, PA.  Construction Cost:  $357,220.00.  The scope of work includes, but is not limited to, demolish buildings #0350101, #0350107 and #0350116 including the passageway between #0350101 and #0350102.  Relocate electrical service from Building #0350101 to Building #0350102.  Install double entrance door in Building #0350102 at location of old passageway.  Construct new restrooms in Building #0350102 – one (1) men’s restroom and one (1) ADA/uni-sex restroom.  Install additional piping and new sewer lateral to service new restrooms.  Relocate and re-wire existing emergency generator.  Replace existing single pane windows; install approximately 250 linear feet of new perimeter fence; install bituminous paving and install new wall-mounted exterior lighting on Building #0305102.

Professional Design Basic Services Fees for this project have been established at 15% of the Construction Allocation.

PROJECT NO. DGS 515-28 PHASE 1 – Renovate Patient Bathroom Including Fixtures to Comply with ADA Accommodations, Wernersville State Hospital, Wernersville, Berks County, PA.  Construction Cost:  $500,000.00.  The scope of work includes, but is not limited to, total and partial renovations of patient bathrooms, shower rooms and dressing rooms located within Patient Wards of Buildings Nos. 35 & 37.  Upgrade the work areas bring the spaces into full compliance with all applicable accessible regulations with eliminating existing barriers and constructing greater clearances.  Renovations shall afford a better environment for staff with providing patient care along with removing suicide hazards by incorporating anti-ligature products and good design practices.

Professional Design Basic Services Fees for this project have been established at 15% of the Construction Allocation.
 

PROJECT NO. DGS 961-32 PHASE 1 – Rehabilitate Readiness Center, Carbondale Readiness Center, Carbondale, Lackawanna County, PA.  Construction Cost:  $1,500,000.00.  The scope of work includes, but is not limited to, repairs to masonry facade (re-point, re-caulk, cleaning & sealing) at both RC and Maintenance Building, replace certain doors and windows on both buildings.  Paint, new floors and ceiling in administrative area and drill hall. Renovate and upgrade men and women toilet rooms, select renovations to kitchen.  Renovate interior space at the maintenance building.  Upgrade lights with controls at both buildings and new site lighting.  Design a 20KW to 30KW photovoltaic system for supplemental power.  Design for a 20KW to 30KW emergency generator.  Complete renovation of HVAC system and equipment, ATC system.  Replace/upgrade heating system in maintenance building.  Replace toilet fixtures throughout, new drinking fountains, replace new main shutoff valves throughout, backflow preventer if required.  Military Equipment Parking area upgrades, new surface and fencing.  Anti-Terrorism/Force Protections upgrades as directed by DMVA. 

Professional Design Basic Services Fees for this project have been established at 10% of the Construction Allocation.

PROJECT NO. DGS 962-29 PHASE 1 – Rehabilitate Readiness Center, Hershey Readiness Center, Hershey, Dauphin County, PA.  Construction Cost:  $1,500,000.00.  The scope of work includes, but is not limited to, repairs to masonry facade (re-point, re-caulk, cleaning & sealing).  Paint, new floors and ceiling in administrative area and drill hall. Renovate and upgrade men and women toilet rooms, select renovations to kitchen.  Upgrade lights with controls at both buildings and new site lighting.  Design a 20KW to 30KW photovoltaic system for supplemental power.  Design for a 20KW to 30KW emergency generator.  Complete renovation of HVAC system and equipment, ATC system.  Replace toilet fixtures throughout, new drinking fountains, replace new main shutoff valves throughout, backflow preventer if required.  Military Equipment Parking area upgrades, new surface and fencing.  Anti-Terrorism/Force Protections upgrades as directed by DMVA. 

Professional Design Basic Services Fees for this project have been established at 10% of the Construction Allocation.

PROJECT NO. DGS 963-54 PHASE 4 – Rehabilitation of Readiness Center, New Castle Readiness Center, New Castle, Lawrence County, PA.  Construction Cost:  $2,405,000.00.  The scope of work includes, but is not limited to, repairs to masonry facade at both RC and Maintenance Building, replace certain doors and windows on both buildings.  May include overhead doors, hollow metal and store front aluminum doors.  Complete renovation of approximately 20% of the administrative space demolition through new construction of space, minor reprogramming of space will occur. Paint, new floors and ceilings in the balance of the areas and reconstruct new men and women toilet room to both buildings.  Renovate kitchen area and new interior doors. Upgrade interior lights with controls to both buildings and new site lighting.  Complete renovation of HVAC system and equipment, ATC system.  Replace/upgrade heating system in maintenance building.  Replace toilet fixtures, new drinking fountains. Military Equipment Parking area upgrades, new surface and fencing.  Anti-Terrorism/Force Protections upgrades as directed by DMVA. 

Professional Design Basic Services Fees for this project have been established at 9.5% of the Construction Allocation.

PROJECT NO. DGS 963-59 PHASE 1 – Rehabilitate Readiness Center, Beaver Falls Readiness Center, Beaver Falls, Beaver County, PA.  Construction Cost:  $1,500,000.00.  The scope of work includes, but is not limited to, repairs to masonry façade at both RC and Maintenance Building, replace certain doors and windows on both buildings.  Paint, new floors and ceiling in administrative area and drill hall. Renovate and upgrade men and women toilet rooms, select renovations to kitchen.  Renovate interior space at the maintenance building. Upgrade lights with controls at both buildings and new site lighting.  Design a 20KW to 30KW photovoltaic system for supplemental power.  Design for a 20KW to 30KW emergency generator.  Complete renovation of HVAC system and equipment, ATC system.  Replace/upgrade heating system in maintenance building.  Replace toilet fixtures throughout, new drinking fountains, replace new main shutoff valves throughout, backflow preventer.  Military Equipment Parking area upgrades, new surface and fencing.  Anti-Terrorism/Force Protections upgrades as directed by DMVA.

  • Project Programs

  • A completed Form ASP-150 (Rev. 05/14), must be received electronically at RA-GSSELECTIONS@pa.gov before the close of business (5:00 p.m.), Thursday, July 17, 2014.  Any other method of delivery will result in the return of the Form ASP-150 to the prospective applicant.  Please note - FORM ASP-150 (Rev. 05/14) must be signed, sent and received by the Department from the company email account of one of the individuals listed in Section 2, Item 14.

 

  • The fee to be paid to the appointed design professional by the Department is indicated within each specific project advertisement.

 

  • The Selections Committee may at its discretion establish interviews with any or all of the Professionals who have requested consideration for appointment as designer for the above projects.  If an interview is required, the Professional will be notified by the Committee as to the date, time and location.  Additional information, in writing, may be requested by the Committee as required.

 

  • Additional Services - Indoor Air Quality Assessment Program and Hazardous Materials
    • The Professional firm selected to design a project will be expected to perform and administer, when required by the Department as Additional Services, an Indoor Air Quality Assessment Program during Building or Renovation Commissioning and the sampling, testing, inspection and monitoring for removal of any asbestos, other hazardous waste or contaminants encountered during project design or construction, unless otherwise stated in the Scope.

 

  • Small Diverse Business Consideration In Professional Selections
    • The Department of General Services encourages the participation of Small Minority Business Enterprises, Small Women Business Enterprises, Small Veteran Business Enterprises and Small Service‐Disabled Veteran Business Enterprises (collectively referred to as “Small Diverse Businesses” as defined under Title 4 of The Pennsylvania Code, Chapter 58) in Commonwealth Design Professional Agreements.
    • Therefore, the Selections Committee on behalf of the Department of General Services will consider participation of Small Diverse Businesses (SDB) as either the prime professional, or as a subcontracting consultant, as a factor in the final selection of Design Professionals on Commonwealth projects.
    • Firms wishing to participate in Commonwealth Design Professional Agreements should specify whether the professional or consultants being proposed on the projects are currently recognized as SDB’s by the Department of General Services, Bureau of Small Business Opportunities. If the proposing firm is itself a current SDB, the firm should state that fact in its proposal.

All applications submitted are subject to review by the Selections Committee.  The Selections Committee disclaims any liability whatsoever as to its review of the applications submitted and in formulating its recommendations for selection.  All recommendations for selection made by the Committee shall be final pursuant to the Commonwealth Procurement Code, 62 Pa. C.S § 101 et seq., as amended (Act 57 of May 15, 1998).

  • Please Note:

    • Design firms submitting Form ASP-150 in response to the above advertised projects must utilize the updated Form ASP-150 (Rev. 05/14), older versions will not be accepted by the Department.
    • No alterations or additions to the Form ASP-150 (Rev. 05/14) will be permitted.  Any deviation may result in rejection.
    • The submission of Form ASP-150 (rev. 05/14) will only be accepted electronically.  Form ASP-150 (Rev. 05/14) submitted by any other means will be rejected and returned.

 

  • Section 1
    • Item 1 – Enter the DGS Project Number as indicated in the advertisement for the specific project the application is being submitted.

  • Section 2
    • Item 4 – Enter firm’s business name.
    • Item 5 – Enter firm’s Commonwealth SAP Vendor ID (Vendor Registration can be accessed at www.pasupplierportal.state.pa.us or by calling 717-346-2676 (Harrisburg Area) or 877-435-7363 (Toll Free).
    • Item 6 – Enter firm’s business street address.
    • Item 7 – Enter firm’s City/State.

    • Item 10 – If firm is a Small Business or Small Diverse Business, enter the firm’s DGS Self-Certification Number; If the firm is a DGS verified Small Diverse Business, enter the firm’s DGS Small Business Self-Certification Certificate Number and check the appropriate Small Diverse Business Type(s).  If firm is not a DGS Self Certified Small Business, enter N/A.

    • Item 15 – List the number of full-time personnel for the specific office of the firm for which the application is being submitted, categorized by Registered, Professional Degree or Other, count each employee only once.
    • Item 16 – Enter up to three (3) consultants that will be retained to assist in the design process, including the consultant’s name, address, number of projects completed together, value of projects completed together, past projects worked together and scope of work to be assigned for the project which the application is being submitted, and registration expiration;  If the consultant is a DGS Self-Certified Small Business, enter the consultant’s DGS Small Business Self-Certification Certificate Number;  If the consultant is a DGS verified Small Diverse Business, enter the DGS Small Business Self-Certification Certificate Number and check the appropriate Small Diverse Business Type(s).
    • Item 17 – Enter up to five (5) individuals who will be assigned to this project, including name, firm name, registration number (if registered professional), registration expiration, physical location while on this project, general work assignment/role on this project, individual’s specialty/discipline, degree earned, year graduated and school or university.

 

  • Section 4
    • Item 18 – Enter the projects the firm holds with the Commonwealth that are currently in Design or Bidding Stage, including project name, agency for which the project is being developed, and final design due date or bid date.
    • Item 19 - Enter the projects the firm holds with the Commonwealth that are currently in Construction, including project name, agency for which the project is being developed, scheduled completion date and current change order rate for the project.
    • Item 20 – Enter up to three (3) projects that best illustrate the applying firm’s qualifications for the specific project the application is being submitted, including project name, location, owner, work being performed by the firm, total value of awarded construction contracts, total value of final construction contracts at project completion, the original scheduled completion date, the actual completion date and any additional pertinent comments within the space permitted.
    • Item 21 – Enter any additional comments or descriptions of relevant information supporting the firm’s qualifications within the space provided. Do not exceed the space limit or attach any additional pages.

 

  • Certification, Signature and Submission
    • By entering firm’s name, signing in the appropriate Business Type fields, either by typing in name, applying a digital signature or printing, applying wet signature and re-scanning and submitting this ASP-150 (Rev. 05/14) you agree that you are signing and submitting this Application on behalf of the firm, and have the authority to do so.
    • The Form ASP-150 (Rev. 05/14) will only be accepted electronically.
    • The Form ASP-150 (Rev. 05/14) is to be emailed to RA-GSSELECTIONS@pa.gov .  Form ASP-150 (Rev. 05/14) submitted by any other means or older versions of Form ASP-150 will not be considered by the Committee and will be returned.
