DESIGN PROFESSIONAL SELECTIONS

FOR

DEPARTMENT OF GENERAL SERVICES

 


The Selections Committee for the Department of General Services will meet to consider selection of Design Professionals for the following projects:

STATEWIDE SURVEYOR SERVICES -  The Department of General Services (DGS) is seeking land surveyors for Surveyor Services in connection with the design and construction of various Commonwealth civil engineering, building, and flood protection projects, as well as real property acquisitions and dispositions throughout the Commonwealth.

PLEASE NOTE:  If your firm has received notification that your firm was selected for the previously advertised Statewide Surveyor Services for the Commonwealth, you need not apply again.

PROJECT NO. DGS 251-53 PHASE 1 – Demolish old Dauphin County Materials Testing Facility – PennDOT, Harrisburg, Dauphin County, PA.  Construction Cost:  $2,000,000.  The scope of work includes, but is not limited to, demolish of structures, including abatement, utility disconnection, final site grading.

PROJECT NO. DGS 376-7 PHASE 1 – Replace Roofs on Dietary Building No. 4 and Housing Unit Building C, State Correctional Institution – Laurel Highlands, Somerset Twp., Somerset County, PA.  Construction Cost:  $640,000.  The scope of work includes, but is not limited to, removal of the existing roof on the Dietary Building.  The top portion is an EPDM Rubber Roof and the Department of Corrections would prefer using a TPO replacement.  The sloped sides of the roof are shingled and DOC would prefer a standing seam which matches recently installed on B & C Blocks.  Please note that the Building C roof has bee replace under a recent project and is not longer within the scope of this project.

PROJECT NO. DGS 513-33 PHASE 1 – Upgrade Existing Coal-Fired Boilers, Torrance State Hospital, Derry Twp., Westmoreland County, PA.  Construction Cost:  $5,550,000.  The scope of work includes, but is not limited to, rehabilitation and or replacement of the existing coal boilers which are in a severely deteriorated condition.  Should new boilers be necessary, the project my require demolition of the existing boilers in the existing boiler plant building or the construction of new steel sided auxiliary to make sufficient square footage available to sight new package boilers.

PROJECT NO. DGS 515-29 PHASE 1 – Repair Breech Mill Dam Breast and Connecting Waterway Conduits, Wernersville State Hospital, S. Heidelberg Twp., Berks County, PA.  Construction Cost:  $1,300,000.  The scope of work includes, but is not limited to, restoration and stabilizing upstream/downstream channels and other surfaces affected by the breach.  Channel restoration, including restoration and stabilization of stream channel, construction the banks with elevated 100 year flood plan along with vegetating and stabilizing silt material in the stream bed.  Seeding and landscaping the areas affected by the breach and relocation and restoration of all affected utilities.

PROJECT NO. DGS 960-55 PHASE 3 – Demolition and Infrastructure Upgrades, Southeastern Veterans’ Center, Spring City, Chester County, PA.  Construction Cost:  $10,678,400.  The scope of work includes, but is not limited to, Part 1, Demolition work: Demolish approximately 4-5 million cubic feet of vacant institutional style buildings, houses, mechanical pump-stations, and other abandon facilities on the campus. Part 2, Rood repair:  Repair and repave the road infrastructure and parking lots on campus.  Improve/develop Storm Water Management (SWM) systems as required to meet current regulations.  Security measures such as fencing and site lighting will be required. Construct breezeway between Coates Hall and the new addition.  Part 3, Replace elevators, replace the roof:  Upgrade electrical, Plumbing and HVAC infrastructure.

PROJECT NO. DGS 961-21 PHASE 3 – Rehab of Existing Readiness Center, Allentown Readiness Center, Allentown, Lehigh County, PA.  Construction Cost:  $3,600,000.  The scope of work includes, but is not limited to, rehab of 41,500 SF of existing Readiness Center, modifications/improvements to heating, electrical, plumbing, IT, Fire Alarm systems. Renovating the bathrooms and shower areas, architectural improvements to the ceiling, walls and flooring, ADA access to the first floor.  Demo of an existing storage building, build a new Unheated Storage Shed and repairs to an existing shed and some minor site improvements.  Meet current L&I and UCC requirements.

PROJECT NO. DGS A 961-38 PHASE 1 – Replacement of Boilers and AHUS, Gino J. Merli Veterans’ Center, Scranton, Lackawanna County, PA.  Construction Cost:  $423,500.  The scope of work includes, but is not limited to, replacement of the boilers.  The boilers are original to the building and are past or very near the end of its project life cycle.  Replacement of Air hander Units (AHU).  The AHU’s are also original to the building and are near the end of its projected life cycles. 

PROJECT NO. DGS 963-55 PHASE 3 – Rehab of Existing Readiness Center, Everett Readiness Center, Everett, Bedford County, PA.  Construction Cost:  $1,000,000.  The scope of work includes, but is not limited to, rehab of 7,500 SF of existing Readiness Center, modifications/improvements to heating, electrical, plumbing, IT, Fire Alarm systems.  Selective demolition and new bathrooms and administrative space, architectural improvements to the ceiling, walls and flooring.  Expand/improve parking for both military and personal vehicles, new storage building.  Meet current L&I and UCC requirements.

PROJECT NO. DGS 963-57 PHASE 1 – Rehabilitate of Readiness Center, Greensburg Readiness Center, Greensburg, Westmoreland County, PA.  Construction Cost:  $2,400,000.  The scope of work includes, but is not limited to, rehab of 17,600 SF of existing Readiness Center, modifications/improvements to heating, electrical, plumbing, IT, Fire Alarm systems.  Renovation the bathrooms and shower areas, architectural improvements to the ceiling, walls and flooring, ADA access to the main floor.  Connect sewer line to public system, remove existing holding tank and back fill.  Site improvements include new military parking area; repave access drive and parking, some repair work to the two unheated storage buildings.  Meet current L&I and UCC requirements.

PROJECT NO. DGS 1572-14 PHASE 1 – Expand the Kitchen Preparation Area and Dining Area, State Correctional Institution – Mercer, Mercer County, PA.  Construction Cost:  $2,500,000.  The scope of work includes, but is not limited to, new construction to expand the inmate dining room, food production and storage areas, as well as renovation of existing food production and storage areas.  The project will include either new or relocation of existing walk-in freezer and refrigeration units, ovens, etc.

INSTRUCTIONS FOR FILING APPLICATION  (Please note, FORM ASP-150 has been updated 11/2012 to reflect changes relative to the implementation of the Small Diverse Business Initiative.  Design Firms submitting ASP-150 Applications for the projects advertised above must utilize the 11/2012 update form, older versions will not be accepted by the Department)

Professionals will not be considered by the Committee until all of the following requirements are met:

(a) Signed Form 150-ASP must be filed with the Department of General Services.  The signature on Form 150-ASP must be an original signature.  The firm’s Vendor Registration number should be listed on Page 1 adjacent their firm name.  Vendor Registration web address is www.pasuppplierportal.state.pa.us or by calling 717-346-2676 (Harrisburg area) or 877-435-7363 (toll free).  Consultants listed on the requesting professional firm's application, Form 150-ASP shall be deemed to be designated Key Consultants.  Form 150-ASP, Application For Specific Project, may be obtained upon request to the Selections Committee, Department of General Services, 3rd Floor Arsenal Building, 18th & Herr Streets, Harrisburg, Pennsylvania 17125.  Telephone:  (717) 783-8468.  The Form 150-ASP can also be downloaded from this Web Site.  In addition, the Form 150-ASP can be obtained via e-mail by addressing a request to:  nspade@pa.gov


(b) Under question 9, page 4 of the requesting professional firm's application (Form 150-ASP), the requesting professional firm shall state, following the consultant’s name, whether the consultant is a Commonwealth Verified Small Diverse Business.  Also, the requesting professional firm shall obtain from each consultant listed, a signed letter of certification on the consultant's letterhead, attesting to the firm's consent to participate in the requesting professional firm's application (Form 150-ASP) for the specific project.  Signed letters of certification from consultants are required and constitute a part of the requesting professional firm's application (Form 150-ASP) for the specific project.  The signature on the letter of certification must be an original signature.


(c) The requesting professional firm must submit six (6) copies of Form 150-ASP, Application for Specific Project, for each project herein advertised in which the firm is interested and qualified to perform.  At least one of the six (6) copies submitted must contain all original signatures.  For architectural projects and, when appropriate, for engineering projects, the Professional shall supply photographs showing a maximum of two different views of each of the three relevant projects described in Question 12, Pages 7 through 9 of the application (Form 150-ASP).  The requesting professional firm or joint venture firm must be the Professional of Record for the projects described in Question 12, Pages 7 through 9 of the application (Form 150-ASP).  It is not acceptable to list work performed by Key Consultants.  The identification and appropriate supportive information concerning each photograph shall include the name, project title, location, and the name of the Professional of Record.  Color photo copies are acceptable, however, renderings and brochures will not be accepted in lieu of photographs.  The photographs shall be attached to each copy of the application, (Form 150-ASP).  The pages of each copy of Form 150-ASP must be stapled with photographs and consultant's letters of certification followed by the resume(s) of Key Personnel (listed in Question 10 and/or 11 on Pages 5 and/or 6) included as the last section of the application.

DO NOT BIND THE APPLICATION (FORM 150-ASP) IN ANY WAY TO ANY OTHER DOCUMENTATION.  DO NOT BIND THE APPLICATION (FORM 150-ASP) IN A BINDER OF ANY TYPE.

  
(d) A complete project submission, which consists of documents described in paragraphs (a), (b) and (c) above, must be received on or before the close of business (5:00 p.m.), Monday, March 25, 2013, and addressed to the Selections Committee, Department of General Services, 3rd Floor Arsenal Building, 18th & Herr Streets, Harrisburg, Pennsylvania 17125.  FAX APPLICATIONS ARE NOT ACCEPTABLE

Note:  Project submissions must be made on the Form 150-ASP.  Applications submitted on any other form are unacceptable and will not be considered by the Committee. 


(e) The Selections Committee may at its discretion establish interviews with any or all of the Professionals who have requested consideration for appointment as designer for the above projects.  If an interview is required, the Professional will be notified by the Committee as to the date, time and location.


(f) Additional information, in writing, may be requested by the Committee as required.


(g) The terms of the Professional’s contract and the fee to be paid to the appointed design professional will be negotiated by the Department for each project.

ADDITIONAL SERVICES - INDOOR AIR QUALITY ASSESSMENT PROGRAM AND HAZARDOUS MATERIALS 

The Professional firm selected to design a project will be expected to perform and administer, when required by the Department as Additional Services, an Indoor Air Quality Assessment Program during Building or Renovation Commissioning and the sampling, testing, inspection and monitoring for removal of any asbestos, other hazardous waste or contaminants encountered during project design or construction, unless otherwise stated in the Scope.

SMALL DIVERSE BUSINESS CONSIDERATION IN PROFESSIONAL SELECTIONS

The Department of General Services encourages the participation of Small Minority Business Enterprises, Small Women Business Enterprises, Small Veteran Business Enterprises and Small Service‐Disabled Veteran Business Enterprises (collectively referred to as “Small Diverse Businesses” as defined under Title 4 of The Pennsylvania Code, Chapter 58) in Commonwealth Design Professional Agreements.

Therefore, the Selections Committee on behalf of the Department of General Services will consider participation of Small Diverse Businesses (SDB) as either the prime professional, or as a subcontracting consultant, as a factor in the final selection of Design Professionals on Commonwealth projects.

Firms wishing to participate in Commonwealth Design Professional Agreements should specify whether the professional or consultants being proposed on the projects are currently recognized as SDB’s by the Department of General Services, Bureau of Small Business Opportunities. If the proposing firm is itself a current SDB, the firm should state that fact in its proposal.

All applications submitted are subject to review by the Selections Committee.  The Selections Committee disclaims any liability whatsoever as to its review of the applications submitted and in formulating its recommendations for selection.  All recommendations for selection made by the Committee shall be final pursuant to the Commonwealth Procurement Code, 62 Pa. C.S § 101 et seq., as amended (Act 57 of May 15, 1998).
