DESIGN PROFESSIONAL SELECTIONS

FOR

DEPARTMENT OF GENERAL SERVICES

 

 

LETTER FROM THE DEPUTY SECRETARY FOR PUBLIC WORKS, DEPARTMENT OF GENERAL SERVICES


 

 

The Selections Committee for the Department of General Services will meet to consider selection of Design Professionals for the following projects:

 

PROJECT NO. DGS 114-5 PHASE 1 – New Visitors Center, Hickory Run State Park, Kidder Twp., Carbon County, PA.  Construction Cost:  $6,400,000.  The scope of work includes, but is not limited to, design and construct new visitor center with administration facilities and comfort station. To include space for visitor orientation, classroom/meeting space, exhibits and display, comfort facilities, administrative offices, and utility upgrades/modifications to suit project.  Design and construct access road, parking and storm water facilities, site/civil and landscaping upgrades to suit project.

Professional Design Basic Services Fee for this project will be negotiated.

PROJECT NO. DGS 141-10 PHASE 1 – Construct New Resource Center with Storage Building, Delaware Canal State Park, Bucks County, PA.  Construct Cost:  $2,750,000.  The Scope of work includes, but is not limited to, design and construct new visitor with administration facilities and comfort stations with space for visitor orientation, classroom/meeting space, exhibits, ranger staff, administrative offices and comfort facilities. Design and construct new storage building, utility upgrades/modification to suit project, design and construct access road, parking and storm water facilities.

The Basic Services fee has been established at 10% of the Base Construction Amount of $2,750,000 for this project.  The Schematic and Programming Design Submissions are to be combined and the Interim Submission is waived.  A maximum of 30 construction phase meetings/site visits and two (2) post-closeout inspection meetings will be included as Base Services.

PROJECT NO. DGS 141-12 PHASE 1 – Rehabilitate Pecks Pond Dam, Delaware State Forest , Porter Twp., Pike County, PA.  Construction Cost:  $6,000,000.  The scope of work includes, but is not limited to, address inadequate capacity of existing spillway and dam structure, upgrade hydraulic capacity of dam and spillway structure to the required probable maximum flood requirements, hydraulic and hydrologic analysis of existing spillway and dam and address aquatic invasive weed problem in Pecks Pond.  Inspect existing concrete outlet culvert and intake structure, underwater inspection of the intake structure and address seepage along right downstream spillway wall.  Rehabilitate or replace outlet structure and replace stoplogs with sluice gate, replace gabion basket stream bank protection along left channel bank below spillway and replace access footbridge below spillway structure.

Professional Design Basic Services Fee for this project will be negotiated.

PROJECT NO. DGS 151-1 PHASE 1 – Rehabilitate Campground Restrooms and Shower Houses with Modern Facilities, Caledonia State park, Franklin Twp., Franklin, County, PA.  Construction Cost:  $2,400,000.  The scope of work includes, but is not limited to, replace existing deteriorated shower houses and comfort facilities and new facilities, provide one additional new shower house at a new location and utility upgrades and modification to suit project including new sewer, water electric utilities at new location.  Provide campsites with full utility hook-up (electric, water and sewer).  Site/civil and landscaping upgrades including access, parking and storm water facilities to suit project.

The Basic Services fee has been established at 10% of the Base Construction Amount of $2,400,000 for this project.  The Schematic and Programming Design Submissions are to be combined and the Interim Submission is waived.  A maximum of 30 construction phase meetings/site visits and two (2) post-closeout inspection meetings will be included as Base Services.

PROJECT NO. DGS 163-36 PHASE 1 – Replace Beach/Shower Houses, Presque Isle State park, Millcreek Twp., Erie County, PA.  Construction Cost:  $2,880,000.  The scope of work includes, but is not limited to, renovations to, provide new beach/shower house facilities and associated amenities around these facilities.  Provide walkways, foot-showers, parking and decks/patios and include food concession space and parking pads for food carts/trailers with full utility hookup (electric, water and sewer).  Utilities upgrades/modifications to suit project including water and sewage upgrades to allow connection to municipal water and sewage for all buildings.  Design and construct access road, parking and storm water facilities and site/civil and landscaping upgrades to suit project.  Demolish existing beach houses.

The Basic Services fee has been established at 10% of the Base Construction Amount of $2,880,000 for this project.  The Schematic and Programming Design Submissions are to be combined and the Interim Submission is waived.  A maximum of 30 construction phase meetings/site visits and two (2) post-closeout inspection meetings will be included as Base Services.

PROJECT NO. DGS 197-40 PHASE 1 – Construction a New Resource Center with Storage Building, Delaware State Forest, Pike County, PA.  Construction Cost:  $7,000,000.  The scope of work includes, but is not limited to, design and construct new resource management center to include space for visitor orientation, exhibits, ranger staff, administrative areas, comfort facilities and meeting space.  Design and construct a new vehicle and equipment storage building, utility upgrades/modifications to suit project.  Design and construct access road, parking and storm water facilities.

Professional Design Basic Services Fee for this project will be negotiated.

PROJECT NO. DGS 197-41 PHASE 1 – New Wood Shop and Sign Shop, Penn Nursery, Centre County, PA.  Construction Cost:  $4,400,000.  The scope of work includes, but is not limited to, design and construct new sign shop and wood shop to include paint spray booth, laminate facilities, computer generated sign fabricator, central vacuum dust collection and separate HVAC systems and utilities upgrades/modifications to suit project.  Site/civil and landscaping upgrades include access, parking and storm water facilities to suit project. Professional Design Basic Services Fee for this project will be negotiated.

PROJECT NO. DGS 200-3 PHASE 1 – Construction of a New Visitor Center, Pymatuning Wildlife Center, Crawford County, PA.  Construction Cost:  $1,600,000.  The scope of work includes, but is not limited to, construct new public building to serve as a wildlife museum/visitor center at Lake Pymatuning.  The main part will house exhibits and displays, auditorium, office space, restrooms and administrative area.

The Basic Services fee has been established at 12% of the Base Construction Amount of $1,600,000 for this project.  The Schematic and Programming Design Submissions are to be combined and the Interim Submission is waived.  A maximum of 30 construction phase meetings/site visits and two (2) post-closeout inspection meetings will be included as Base Services.

PROJECT NO. DGS 960-81 PHASE 1 – Renovations and General Repairs, Delaware Valley Veterans’ Home, Philadelphia, Philadelphia County, PA.  Construction Cost:  $4,500,000.  The scope of work includes, but is not limited to, renovation to all 171 resident rooms and nursing stations, to install vinyl wainscoting in all resident rooms and at nursing station areas and install a total of 141 medicine cabinets in resident rooms for skilled nursing residents.  Renovate C-wing dining room by opening wall between pantry and dining room to improve dietary services.  Replace and upgrade doors, replace parking lot and driveway lighting with LED lights and construct new concrete walkway along facility main driveway.  Purchase and install new LED TV’s and remove existing carpet and install new carpet for facility admin. Suite and the south end of Main Street area

Professional Design Basic Services Fee for this project will be negotiated.

PROJECT NO. DGS 960-82 PHASE 1 – Renovations and General Repairs, Pennsylvania Soldiers’ and Sailors’ Home, Erie, Erie County, PA.  Construction Cost:  $6,423,000.  The scope of work includes, but is not limited to, renovations to the Personal Care Building to establish new storage (goods) area on the third floor and installation of new freight elevator.  Upgrade PA System and installation of 100% wireless capability (coverage) in support of Department’s new electronic medical records and clinical/business operations and refurbish/rebuild existing boilers.  Renovation to the nursing care unit resident rooms (75) and bathrooms.  To include windows, refurbish coat block walls, over the bed lighting, paint, hallway cameras, exterior door locking system, exterior cameras, and others as appropriate.

Professional Design Basic Services Fee for this project will be negotiated.

PROJECT NO. DGS A 962-55 PHASE 1 – Renovations and General Repairs, Hollidaysburg Veterans’ Home, Hollidaysburg, Blair County, PA.  Construction Cost:  $819,672.13.  The scope of work includes, but is not limited to, replacement of automatic transfer switch at Arnold Hall and replacement of all emergency panels that are supported by the automatic transfer switch.  Replacement of main electrical panel at Old Administration Building.  Replacement of roof at the Old Administration Building, removal/demolition of existing roofing materials, installation of replacement decking and new underlayment, installation of new shingles, and installation of new gutters/downspouts, flashing and drip edge.

The Basic Services fee has been established at 14% of the Base Construction Amount of $819,675.13 for this project.  The Schematic and Programming Design Submissions are to be combined and the Interim Submission is waived.  A maximum of 20 construction phase meetings/site visits and two (2) post-closeout inspection meetings will be included as Base Services.

PROJECT NO. DGS 963-204 PHASE 1 – Renovations and General Repairs, Gino J. Merli Veterans’ Home, Scranton, Lackawanna County, PA.  Construction Cost:  $4,011,000.  The scope of work includes, but is not limited to, epoxy paint floor to dietary kitchen and loading dock.  Renovate nurses stations on first, second, and third floors and installation of new cameras for safety and security.  Enlarge three auto doors and upgrade main entrance and exit steel door and frames.  Expansion of existing maintenance shop in support facility’s operations,  Interior carpet renovation due to the unavailability of replacement carpet for repairs, replace with sheet flooring design throughout facility.  Renovate toilet rooms and community bathrooms.  Construct new retaining wall 100’x24” and concrete steps in parking lot.

Professional Design Basic Services Fee for this project will be negotiated.

PROJECT NO. DGS 967-53 PHASE 1 – Renovations and General Repairs, southwestern Veterans’ Center, Pittsburgh, Allegheny County, PA.  Construction Cost:  $1,600,000.  The scope of work includes, but is not limited to, facility wide duct cleaning, replacement of bathroom exhaust fans, and rebuild air handlers (09) and replace caulking around all window and door facility wide.  Installation of additional cameras (15) to current CCTV system and installation 100% wireless capability (coverage) in support of Department’s new electronic medical records and clinical/business operations.

The Basic Services fee has been established at 12% of the Base Construction Amount of $1,600,000 for this project.  The Schematic and Programming Design Submissions are to be combined and the Interim Submission is waived.  A maximum of 30 construction phase meetings/site visits and two (2) post-closeout inspection meetings will be included as Base Services.

PROJECT NO. DGS 971-10 PHASE 1 – Upgrade HVAC Systems with Geothermal Systems, Pennsbury Manor, Morrisville, Bucks County, PA.  Construction Cost:  $3,000,000.  The scope of work includes, but is not limited to, reduce the energy usage of the existing buildings, to provide better climate control in the Manor House in order to improve the collections conservation, and to upgrade old utility lines.  Replace and/or configure the existing systems with geothermal systems and other energy-savings equipment.  The utility lines upgrades will focus on old utility lines, primarily water distribution, that have not been replaced under past infrastructure upgrade projects.  Additionally, the Manor House is an important historical resource on the site, so the HVAC upgrades will necessitate the need for a historical design professional with experience working on similar projects.  Asbestos containing material will likely be encountered in the Manor House during this project and should be abated.  Archaeological investigations will need to be conducted for any earthwork disturbance including utility line upgrades or geothermal systems installations.

The Basic Services fee has been established at 10% of the Base Construction Amount of $3,000,000 for this project.  The Schematic and Programming Design Submissions are to be combined and the Interim Submission is waived.  A maximum of 30 construction phase meetings/site visits and two (2) post-closeout inspection meetings will be included as Base Services.

PROJECT NO. DGS 972-9 PHASE 1 – Renovation of Utility Infrastructure and Upgrade HVAC System, Landis Valley Museum, Manheim Twp., Lancaster County, PA.  Construction Cost:  $7,500,000.  The scope of work includes, but is not limited to, a variety of improvement to site facilities and infrastructure including the renovation of roads, parking lots, storm water management systems, pedestrian paths, and utility infrastructure (sewer, water, electrical, data).  Also included are energy conservation improvements (including multiple geothermal HVAC systems) and accessibility improvements to various buildings, including upgrades and expansion of restroom facilities.

Professional Design Basic Services Fee for this project will be negotiated.

  • A completed Form ASP-150 (Rev. 05/14), must be received electronically at RA-GSSELECTIONS@pa.gov before the close of business (5:00 p.m.), Tuesday, August 26, 2014.  Any other method of delivery will result in the return of the Form ASP-150 to the prospective applicant.  Please note - FORM ASP-150 (Rev. 05/14) must be signed, sent and received by the Department from the company email account of one of the individuals listed in Section 2, Item 14.

 

  • The fee to be paid to the appointed design professional by the Department is indicated within each specific project advertisement.

 

  • The Selections Committee may at its discretion establish interviews with any or all of the Professionals who have requested consideration for appointment as designer for the above projects.  If an interview is required, the Professional will be notified by the Committee as to the date, time and location.  Additional information, in writing, may be requested by the Committee as required.

 

  • Additional Services - Indoor Air Quality Assessment Program and Hazardous Materials
    • The Professional firm selected to design a project will be expected to perform and administer, when required by the Department as Additional Services, an Indoor Air Quality Assessment Program during Building or Renovation Commissioning and the sampling, testing, inspection and monitoring for removal of any asbestos, other hazardous waste or contaminants encountered during project design or construction, unless otherwise stated in the Scope.

 

  • Small Diverse Business Consideration In Professional Selections
    • The Department of General Services encourages the participation of Small Minority Business Enterprises, Small Women Business Enterprises, Small Veteran Business Enterprises and Small Service‐Disabled Veteran Business Enterprises (collectively referred to as “Small Diverse Businesses” as defined under Title 4 of The Pennsylvania Code, Chapter 58) in Commonwealth Design Professional Agreements.
    • Therefore, the Selections Committee on behalf of the Department of General Services will consider participation of Small Diverse Businesses (SDB) as either the prime professional, or as a subcontracting consultant, as a factor in the final selection of Design Professionals on Commonwealth projects.
    • Firms wishing to participate in Commonwealth Design Professional Agreements should specify whether the professional or consultants being proposed on the projects are currently recognized as SDB’s by the Department of General Services, Bureau of Small Business Opportunities. If the proposing firm is itself a current SDB, the firm should state that fact in its proposal.

All applications submitted are subject to review by the Selections Committee.  The Selections Committee disclaims any liability whatsoever as to its review of the applications submitted and in formulating its recommendations for selection.  All recommendations for selection made by the Committee shall be final pursuant to the Commonwealth Procurement Code, 62 Pa. C.S § 101 et seq., as amended (Act 57 of May 15, 1998).

    • Item 17 – Enter up to five (5) individuals who will be assigned to this project, including name, firm name, registration number (if registered professional), registration expiration, physical location while on this project, general work assignment/role on this project, individual’s specialty/discipline, degree earned, year graduated and school or university.

    • Item 18 – Enter the projects the firm holds with the Commonwealth that are currently in Design or Bidding Stage, including project name, agency for which the project is being developed, and final design due date or bid date.
    • Item 19 - Enter the projects the firm holds with the Commonwealth that are currently in Construction, including project name, agency for which the project is being developed, scheduled completion date and current change order rate for the project.
    • Item 20 – Enter up to three (3) projects that best illustrate the applying firm’s qualifications for the specific project the application is being submitted, including project name, location, owner, work being performed by the firm, total value of awarded construction contracts, total value of final construction contracts at project completion, the original scheduled completion date, the actual completion date and any additional pertinent comments within the space permitted.
    • Item 21 – Enter any additional comments or descriptions of relevant information supporting the firm’s qualifications within the space provided. Do not exceed the space limit or attach any additional pages.

 

  • Certification, Signature and Submission
    • By entering firm’s name, signing in the appropriate Business Type fields, either by typing in name, applying a digital signature or printing, applying wet signature and re-scanning and submitting this ASP-150 (Rev. 05/14) you agree that you are signing and submitting this Application on behalf of the firm, and have the authority to do so.
    • The Form ASP-150 (Rev. 05/14) will only be accepted electronically.
    • The Form ASP-150 (Rev. 05/14) is to be emailed to RA-GSSELECTIONS@pa.gov .  Form ASP-150 (Rev. 05/14) submitted by any other means or older versions of Form ASP-150 will not be considered by the Committee and will be returned.
