This website provides quick access to information about the many long term living services and planning resources available for people over the age of sixty and adults with physical disabilities throughout Pennsylvania.

Whether you are an advocate, a provider or a consumer trying to determine your needs, or beginning to plan for your future, you will find helpful information at your fingertips.

SPECIAL INITIATIVES

Today, many exciting initiatives related to Long Term Living are underway. This site will provide information and updates on activities related to transitioning out of nursing facilities and other forms of institutional living; the creation of easier access to long term living services; efforts to expand consumer directed services and more. Below are links to some of those initiatives.

If you are receiving services through the Aging Waiver or Attendant Care Waiver, you may be able to participate in a new and innovative service model where you can design your own spending plan in order to purchase goods and services related to a need identified on your individual service plan. It’s called Services My Way (SMW).
