Form PDE 338B is for Pennsylvania certified educators and public school entities to use to convert a BRIDGE certificate to an instructional certificate for Pennsylvania certified educators who have fully completed all requirements of the BRIDGE program.
The completed form must include the signature of both the applicant and school entity's superintendent or chief school officer (or designee).
The applicant must submit the required $100 fee (signed US money order) with the completed application form in order to initiate the PDE application review process.
