A. Asbestos Abatement Notification

The Pennsylvania Department of Environmental Protection (PA DEP) regulates the removal of asbestos containing material (ACM).

B. Asbestos Occupations Certification: Initial and Renewal

Please read and follow the directions for each category listed below.
 
All but the contractor certification application forms require the applicant to include her/his latest training information and the certificate for this training. Applications that lack this information and the course certificate will not be processed.
 
Applicants must also provide information pertaining to any enforcement actions taken against him/her.
 
All applicants must pay the specified certification fees, unless they are federal or state government employees or work for a not-for-profit agency.
 
For Contractor Certification:
  • Complete a copy of Asbestos Contractor Certification Application (LIBI-613). Click here to obtain a copy.
     
  • Provide payment of $50.00 via check or money order made payable to the Commonwealth of Pennsylvania. (Same fee applies to initial or a renewal certification.)
     
  • Mail these items to:

Note: This application must contain (on page 2) the certification number and name of an individual in this company that has been certified in the category “Contractor (Individual)” by the Department of Labor & Industry. The contracting firm will not be certified if an employee in the company has not been so certified.

  • Complete pages 1 and 2 of Application For Asbestos Occupation Certification (LIBI-607). Click here to obtain a copy.
     
  • Provide a copy of the certificate for your most recent EPA or PA approved training course.
     
  • Provide information regarding any enforcement actions taken against you, following directions on page 2 of form LIBI-607.
     
  • Provide payment required, following the directions on page 3 of form LIBI-607, via check or money order made payable to the Commonwealth of Pennsylvania.
     
  • Mail these items to:

C. Asbestos Training Accreditation And Other Requirements

Training providers have a number of responsibilities that must be met, to offer courses related to asbestos abatement. These include:
  • Obtaining Department of Labor & Industry accreditation.
     
  • Notifying the Department when a training course will be offered.
     
  • Ensuring that all training participants have signed in on the Department’s form, and submission of this form to the Department.
     
  • Completing an evaluation of each training participant and submitting this to the Department (within 5 days of completion of the training).
Detailed information about complying with each of these requirements follows.
OBTAINING ACCREDITATION:
 
Before offering any asbestos abatement training courses, providers must obtain Department of Labor & Industry accreditation.
 
To obtain accreditation, the following must be submitted:
  1. Pages 1 and 2 of the Department’s Asbestos Training Course Accreditation Application (LIBI-603), with all requested information filled in. Click here to obtain a copy.
     
  2. Payment of the required fees ($1000.00 per course, initial or refresher) via check or money order made payable to the Commonwealth of Pennsylvania.
     
  3. Copies of supporting documentation for each course previously accredited by the U.S. Environmental Protection Agency or another state.
     
  4. Complete information on the content of each course for which you are seeking Pennsylvania accreditation. The following must be included (in addition to the instructor information on page 2):
    1. The curriculum, including:
      1. Length of training (in days).
      2. Topics covered.
      3. Amount and type of hands-on training.
      4. Length, format and passing score for examination(s).
       
    2. Copies of all course materials including student manuals, instructor handbooks, handouts and examinations.
       
    3. Description of how the examination was developed.
       
    4. Sample of the certificate issued to trainees upon the successful completion of the course.

The Department of Labor & Industry will process accreditation applications within 30 days of their receipt. If the package is acceptable, the applicant will be notified of our contingent approval and an on-site visit scheduled to further evaluate the applicant. Only after this onsite visit is made will a Certificate of Accreditation be issued, after which the applicant may begin offering asbestos abatement training courses.
 
NOTIFICATION OF COURSES TO BE OFFERED:
 
Whenever an accredited training provider intends to offer a course, it must submit a completed copy of the Asbestos Training Course Notification Form (LIBI-605) to the Department of Labor & Industry. Click here to obtain a copy of this form.
 
This form should be faxed to 717-705-0196.
 
SIGN-IN SHEET FOR TRAINING PARTICIPANTS:
 
On each day of a scheduled training, the provider must have all participants sign in (for both morning and after sessions, as applicable) on the Department of Labor & Industry’s Asbestos Trainee Sign-In Sheet (LIBI-610). Click here to obtain a copy of this form.
 
Immediately following the training, the sign-in sheet should be faxed to the Department of Labor & Industry at 717-705-0196.
 
TRAINEE EVALUATIONS:
 
Within five (5) days of completion of the training, the provider must submit an evaluation of each participant using the Department of Labor & Industry’s Asbestos Trainee Evaluation Form (LIBI-606). Click here to obtain a copy.
 
Note: Trainees must fill in information and provide their signatures at the top of this form, so providers must distribute and collect these forms before trainees leave their courses.
