The Bureau of Family Health values the relationship it has with providers.  That's why we are providing you with quick and easy online resources - so you can get the information you want and need. This user friendly online tool will eliminate the need for lengthly telephone inquiries and needless mailings of large documents.  Please peruse the various links and resources on this site. 
 

Site Update - 3/2012

Existing Providers - Approximately three months prior to the expiration of your current PPA, you will be notified, via letter, of your new PPA number and your new effective and termination dates.  You will need this information to complete the PPA signature page.

New Providers - Please view the Contact Us list and select the appropriate Provider Type and contact the Program Staff.  Program Staff will provide you with your new PPA number and your effect and termination dates.  You will need this information to complete the PPA signature page.
