NOTE: To allow sufficient time for taxpayers to receive automatically mailed state tax forms, requests for personal income tax forms by mail will not be considered until January 11, 2013. If you have not received the forms you need by January 11, please then submit a request for forms by mail.

Tax-related questions should be made through our Online Customer Service Center. Federal (IRS) forms should be obtained from the IRS.
