IMPORTANT REMINDER!
Beginning with the second quarter of 2014, any calendar week in which an
employee earns $116 or more must be reported as a credit week for the
employee. If the employee earns less than $116, do not report that
calendar week as a credit week.

Following is a list of forms pertaining to employer reporting for Unemployment Compensation (UC). If an alternate version of a form is needed but not available, please contact the UC Employer Contact Center at 1-866-403-6163.

Several of the forms below are Acrobat files that employers can download and fill in at their computer. Unless otherwise stated, Acrobat Reader Version 4.0 or later provides the best results. These forms can be completed in several easy steps:
  • Open the PDF form in Acrobat Reader. If you wish to save the blank form to your hard drive and open it at a later time, you should use the "Save a Copy" button within the Adobe application instead of the "Save" button on your Internet browser.
  • Once the form is open, click on the desired entry field and a blinking cursor appears.
  • After the information has been entered in the entry field, tab to the next field.
  • After all the fields have been completed, print out the page.
  • Completed PDF forms cannot be saved or attached to e-mail messages.
NOTE: Users with Adobe Acrobat Professional as opposed to the Free Acrobat Reader, CAN save entered data for later use as well as submit completed forms via e-mail.

WAGE REPORTING

NOTE: For tips on filing or obtaining Form UC-2/2A, refer to Tips of Completing Reports. Do not file a paper copy of Form UC-2 and/or UC-2A if that form is being submitted using an electronic filing method. This action could result in a duplication of records.
 
UC-2: Employer's Report for Unemployment Compensation
This form is used to report an employer's quarterly gross and taxable wages, and Unemployment Compensation contributions due. It may be completed on your computer, printed and mailed to the Department, however, you must have Adobe Acrobat Reader 6.0 to use it. For security reasons, this form cannot be transmitted over the Internet. If you wish to file your quarterly UC reports online, please click here.
 
 
NOTE: This downloadable form should not be used to prepare facsimiles of PA Form UC-2.
 
UC-2A (REV 7-12): Employer's Quarterly Report Of Wages Paid To Each Employee [Size=1m]
This form is used to list employees' social security numbers, names, gross wages earned, and credit weeks for a particular quarter (3-month period). This is an Acrobat file that can be downloaded and filled in at a computer. To print, please use the print button within Acrobat and if possible, in color.
 
Note: If you would like the system to keep a record of your UC returns and payments so you may view your filing history online, you should file electronically via UCMS.
 
Please Note: Online Filing of Form UC-2A is available using UCMS.

UC-2A Supplement: To Employer's Quarterly Report Of Wages Paid To Each Employee [Size=104k]
This form is used to list employees social security numbers, names, gross wages earned, and credit weeks for a particular quarter (3-month period).
 
UC-181: Electronic Filing Requirements Waiver Request for Pennsylvania Unemployment Compensation Wage Reporting
This form must be submitted if you are currently unable to comply with the electronic filing requirements and are requesting a temporary waiver allowing you to submit your quarterly wage and tax reports via paper Forms UC-2A and UC-2. Mail this completed form to: Office of UC Tax Services, Central Operations Division, Room 914, 651 Boas Street, Harrisburg, PA 17121
 
UC-2B: Employer's Report of Employment and Business Changes [Size=319k]
This form is used to report any recent change in name, mailing address, or business location. It is also used to inform the Office of UC Tax Services that an employer no longer has employees or that a business has been sold or discontinued. This form should be mailed to: Office of UC Tax Services, Tax Accounting Administration, 651 Boas Street, PO Box 60848, Harrisburg, PA 17106-0848.
 
UC-2INS: (UC-2/2A/2B Instructions)
These are the instructions for completion of the UC quarterly tax forms.
 
UC-2X: Pennsylvania UC Correction Report [Size=60k]  
This form is used to make changes to Gross and/or Taxable wages (increase or decrease) from those wages reported on the original PA Form UC-2. Send completed forms to Office of UC Tax Services, 651 Boas Street, PO Box 60848, Harrisburg, PA 17106-0848.
 
UC-2AX: Corrected Pennsylvania Gross Wages Paid to Employees [Size=46k]
This form is used to correct wage records or credit weeks from that reported on the original PA Form UC-2A. Send completed forms to Office of UC Tax Services, 651 Boas Street, PO Box 60848, Harrisburg, PA 17106-0848.
 
UC-252: Replacement UC-2A For Partial Transfer [Size=69k]
This form is used to list employees names, social security numbers, gross wages, and credit weeks when an employer is transferring part of his accumulated experience to another business.

UC-8: Application To Elect Coverage Under the PA UC Law For Exempt Service And Exempt Employers [Size=131k]
This form is used by employers to voluntarily elect coverage under the UC Law.
 
UC-8A: Application To Elect Coverage Under the PA UC Law For Services Performed Entirely in Another State [Size=607k]
This form is used by employers to voluntarily elect coverage under the UC Law, even though services are performed entirely in another state. This is a downloadable form that can be completed on your computer, printed and mailed to the Department.
 
UC-55: Employer's Status Report [Size=377k]
This form is completed by the employer and used by the Office of UC Tax Services to determine the proper state of coverage for multi-state employees. UC-111: Pennsylvania Employee Leasing Questionnaire [Size=2m]
This form is completed by businesses in the employee leasing service industry where there is a question as to the proper reporting of employees leased to the client(s) from the employee leasing service provider. The form can also be used to aid the department in determining the correct business classification of the employee leasing business in cases where the employee leasing business feels it is improperly classified. Note: The Act 5 of 2005 amendments to the PA UC Law now require all employees of client employers of an employee leasing business be reported under the Unemployment Compensation account of the client.
 
RC-1: Employer's Election To Cover Multistate Workers Under The PA UC Law [Size=69k]
This form is used to allow an employer to have the wages of those employees who customarily work in two or more states be reported to Pennsylvania. It can be completed on your computer, printed and mailed to the Department.
 

NONPROFITS

UC-1681: Surety Bond [Size=30k]
This form is a contract/bond between a reimbursable employer's insurer and the Department of Labor and Industry. It serves as collateral in the event UC benefit charges are unpaid and will be liquidated up to the stated amount of the contract/bond. This is a downloadable form that can be completed on your computer, printed and mailed to the Department.
 
You must also complete Form UC-1692, which is used to determine the amount of the contract/bond.
 
UC-1692: Election Or Re-election Of Reimbursement [Size=425k]
This form is the agreement an employer must complete to elect reimbursable coverage. You may also wish to download the form UC-1681.
 
UC-1692A: Election Of Reimbursement - Political Subdivision [Size=425k]
This form is the agreement a political subdivision must complete to elect reimbursable coverage.
 

MISCELLANEOUS

UC-10: Pennsylvania Unemployment Compensation Payment Plan [Size=483k]
This form is used by an employer to enter into a payment plan with the Office of UC Tax Services. The normal length of an approved payment plan is 6 months. This form can be completed on your computer, printed and mailed to the Department.
 
UC-46(A): Petition for Appeal [Size=100k]
Complete Section 1 of this form when appealing a UC Referee Decision to the UC Board of Review.
 
UC-46(B): Petition for Appeal [Size=79k]
Complete Section 1 of this form when appealing a UCSC, Employer Charge determination.
 
UC-140E: Request to Change EFT Payment Method and/or Bank Information [Size=33k]
This form is used to change the banking information or payment method used with e-TIDES. It is a downloadable form that can be completed on your computer, printed and faxed or mailed to the Department.
 
UC-746: Records to be Kept by Employers[Size=33k]
This form provides information for employers on the required content, location and retention of employment records.
 
UC-1208: PA Unemployment Compensation (UC) Benefits Address Confirmation and Power of Attorney
This power of attorney, when properly executed, will permit the attorney-in-fact to represent the employer before the Office of UC Benefits Policy and the Office of UC Service Centers in matters relative to UC benefits under the Pennsylvania UC Law, and to receive designated correspondence on behalf of the employer.
 
UC-1609P(INS): Instructions for Employers When Completing Form UC-1609P How to Apply for Unemployment Compensation (UC) Benefits
 
UC-1609P: Employer Information Form [Size=153k]
This form provides accurate information for use when unemployment claims are filed. If wrong information is given on a claim, it can create delays that can lead to a wrong financial determination, which could increase your tax rate. UC-1921W: Employer Questionnaire Refusal Suitable Work
This form is used to help improve payment accuracy. Refusing an offer of suitable work/referral to work must be reported to the department within seven days of the offer. Use this downloadable form to notify the department of a refusal of suitable work offer or referral to suitable work.
